  • Medical History

The above list includes the important medical conditions relevant to gender-affirming care. If you have a different condition that is not listed then please look it up on our Knowledge Base and if you want to discuss with a doctor then please book an appointment.

  • About Your Current Gender-Affirming Medication

Please pick the gender-affirming medication you are currently taking (if any) so that we can make the most appropriate recommendations for you. Please take care and be as accurate as possible. If your exact prescription is not listed please choose the closest match.

  • Alert - if you are not currently taking any gender-affirming medication, please go back and choose the option "I am wanting / waiting to start gender-affirming care or transfer from another service" at the beginning of the form.

  • Alert - it seems you have chosen to be prescribed both feminising and masculinising hormones - please book an appointment to discuss your transition with a doctor

  • Your Choice of Medication

The next sections ask you which medication you would like to be prescribed. Please choose carefully as some of the medicines are specific for the country you live in. If you are not sure then choose one of the options at the top of the list that are more general, like tablet or patch or gel or injection.

Choose the medication which you would like to be recommended for or prescribed on this occasion. If you are not sure about the different types of medicine and formuation then please book an appointment to speak to someone via the links on our website.

  • Estrogen Hormone

Taking estrogen will maintain or induce feminisation - including breast growth and development, softer skin and fat redistribution causing curves and hip shape changes.

By choosing to be recommended estrogen you are agreeing that you are fully informed about these changes, if you are not please book to speak to someone via the links on our website.

Please make sure you know which oestrogen preparations and brands are available in your country before making your choice. Some of them are only available in some countries. If you are not sure then choose the generic tablet / patch / gel.

  • Alert!

You have indicated that you would like to be prescribed estrogen but you have also said that you have an undiagnosed breast lump or a history of breast cancer.

Estrogen is not suitable for you at this time.

  • Alert!

You have indicated that you would like to be prescribed estrogen via injection but estrogen injections are not available in the UK, please choose another preparation.

  • Progesterone Hormone

Taking progesterone will support or induce feminisation - including breast growth and development, softer skin and fat redistribution causing curves and hip shape changes.

By choosing to be recommended progesterone you are agreeing that you are fully informed about these changes, if you are not please book to speak to someone via the links on our website.

Please make sure that you choose the progesterone that is available in your country as they are not all available in every country.

  • Alert!

You have indicated that you are allergic to soya.

Please choose to have your progesterone by rectal suppository.

  • Testosterone Hormone

Taking testosterone will maintain or induce masculinisation - including growth of facial and body hair, deepening of the voice and increase in muscle mass.

By choosing to be recommended testosterone you are agreeing that you are fully informed about these changes, if you are not please book to speak to someone via the links on our website.

  • Alert!

You have a nut allergy so Sustanon is not a suitable medication for you to take.

Sustanon is not suitable for people who have nut allergies.

Please chose an alternative testosterone preparation.

  • Alert!

You have said that you want to be prescribed tesosterone but have not chosen a preparation.

Please chose an appropriate testosterone preparation.

  • GnRHa Blockers

Taking GnRHa blockers will completely stop your natural hormone production.

By choosing to be recommended GnRHa blockers you are agreeing that you are fully informed about these changes, if you are not please book to speak to someone via the links on our website.

  • UK Government Ban

Sadly, puberty blockers are not currently able to be dispensed in the UK for people under the age of 18. If the application for this recommendation has a date of birth after 2006, or if you are able to take a paper prescriptionn to a chemist outside the UKL then that would be an option for you. Otherwise we recommend choosing a different blocker in the next section - either spironolactone or cyproterone to suppress the effects of testosterone, or raloxifene or testosterone monotherapy to reduce estrogen.

  • Injections

If you want to self-administer your injection, we can arrange for you to have a training session.

Please choose to add this to your basket at the end of this form and we will contact you to book your session.

We would only recommend injections to be prescribed for you if you have had this injection training session or have someone who can administer injections for you.

  • Hormone Blockers - spironolactone, cyproterone, raloxifene

Spironolactone and cyproterone reduce the effects of testosterone on the body. They can be used by people of any age. The Australian guidelines for children recommend them as alternatives if GnRHa blockers are not available.

Spironolactone is best avoided if you have heart disease or are taking blood pressure medication and if you want to be recommended for spironolactone you will need to give us a blood pressure reading.

Cyproterone is not prescribed in people with liver disease.

Raloxifene reduces the action of estrogen on the breasts and womb and can therefore prevent breast growth and menstruation. It can be used in trans masculine children who cannot get access to GnRHa blockers and who are not yet ready for testosterone puberty. It is also used in transfeminine or androgynous people who wish to try and limit breast development associated with estrogen.

  • If you want to be recommended for Spironolactone we need a recent blood pressure reading taken within the last month.

  • Alert!

Due to your medical history Spironolactone is not a suitable antiandrogen.

Spironolactone is not suitable for people who:

    • Are taking medication for high blood pressure or heart disease - because of the risk of high potassium levels and low blood pressure
    • Have Addison's disease or kidney disease

Please go back and choose an alternative antiandrogen.

  • Alert!

Due to your medical history Cyproterone is not a suitable antiandrogen, Cyproterone is not suitable for people who have liver disease.

Please go back and choose an alternative antiandrogen.

  • Alert!

Your blood pressure is too low for it to be safe for you to be prescribed Spironolactone.

Please go back and choose an alternative antiandrogen.

  • Finasteride / Dutasteride

Taking finasteride / dutasteride will reduce the effects of your testosterone, particlualry with regards to hair loss. They are also effective as the sole antiandrogen in some trans feminine people.

By choosing to be recommended finasteride / dutasteride you are agreeing that you are fully informed about these changes, if you are not please book to speak to someone via the links on our website.

  • Alert - Medication Choices

It looks like you haven't chosen any medication to be recommended. If you do not know which to choose, please book an appointment with a doctor via our website so they can help you understand the different medications.

If this was an error, please go back and make your choices.

  • Treatment Recommendations and Independent Prescribers

We will send you a bespoke Treatment Recommendation to outline the recommendations that fit with the best practice and International guidelines across the world.

If your doctor has agreed to prescribe for you, give your Treatment Recommendation to them to advise them what the recommended medication would be and what blood tests you need and when.

If your doctor cannot help you then we can refer your Treatment Recommendation to an Independent Prescriber who is experienced in gender-affirming care and confident in prescribing in this area.

Please choose from the following options.

Note: if you opt for a Treatment Recommendation this means you will be asking your doctor to prescribe for you. If you need a prescription then please choose 'please refer me to an Independent Prescriber.'
